BossArea, I couldn't upload it to the site so I posted it on my personal web for yas. Copy it down, scale & split it as you see fit. I suppose I should have taken the pic on a white background to match your others. Let me know when you've got it.

Hi DeFrag Sorry man, I assumed, that you had gone to all the trouble, to scan the entire booklet.
Well to tell you the truth, I had hoped that it was a scan of the booklet.

this is a link i have to the boss guide,it gives you some sample settings and their take on what pedals you can use in tandem ..ie ds1 and od3 etc....some of the advice is common sense if you have been playing a while but i think its a pretty good book......hope this helps.......
