If diyguitarist.com isn't offering the etched board, or linking you to someone sho is, you'll probably have to etch one yourself (or use point to point wiring, which probably wouldn't be recommended).
You can get components online from Small Bear Electronics, Mouser Electronics, Digikey, etc. Googling on these names should get you to their web sites.
